Summary

  • Job Summary
    • The Care Transitions Navigator maintains ongoing contact with health care organization, Insurance companies, and medical community to facilitate home care services, Specialty Medications, financial assistance programs for medication assistance, and streamlines the transition of patient care between facilities and care providers within CoxHealth 340B clinics. The Navigator will work as a liaison between patients, clinic staff and the Prior Authorization Team.
  • Job Requirements
    • Education
      • Required: Graduate of an approved nursing program
    • Experience
      • Preferred: Minimum one (1) year experience as professional nurse within the last three (3) years.
      • Preferred: Six (6) months home health experience.
    • Skills
      • Knowledge of Medicare and Medicaid regulations desirable.
    • Licensure/Certification/Registration

      • Required: MO RN License

        Required: BLS must be obtained within 90 days
