Provides in-home evaluation and application of Medical Social Services to resolve and/or minimize adverse social and emotional factors related to illness.
Education
Required: Masters Degree from school of Social Work accredited by the Council on Social Work Education.
Experience
Preferred: One (1) year social work experience in home care setting.
Skills
Ability to provide reliable transportation as required by nature of position.
Licensure/Certification/Registration

Eligible to sit for, and successfully pass the test of certification as a certified C ase Manager (C C M) or Accredited C are Manager (AC M) preferred
