Job Summary

• The Health Navigator assists with managing Primary Care Health Home (PCHH) patients by demonstrating a patient-centered approach for coordinating the healthcare needs and wellness goals of the PCHH program patients. This position coordinates non-medical, social needs such as transportation, financial assistance, scheduling appointments, etc. for all patients enrolled in PCHH. In addition to manages the clerical needs of the program including processing enrollment and discharge paperwork to the state, managing multiple program spreadsheets, and processing state reports as received. 

Job Requirements

• Education
  • Required: High School Diploma or Equivalent
• Experience
  • No previous experience required
  • Preferred: Previous experience with Electronic Health Records and /or 2-3 years experience working in a Family Practice out-patient setting
• Skills
  • Excellent telephone and interpersonal communication skills
  • Ability to multi-task and prioritize; strong organizational skills
  • Strong knowledge of medical terminology
  • Computer experience required
  • Evaluation of patient, Care plan oversight and Care Coordination required.
