Job Summary

This position is responsible for facilitating smooth care transitions from an inpatient setting to the appropriate next level of care.  Must ensure an organized, interdisciplinary team approach to developing a quality plan of care for identified patient populations in relation to various programs. The Transition Coordinator will provide educational support to patients/caregivers at bedside and after discharge from the inpatient setting during the transition period. The Transition Coordinator will facilitate timely resolutions to patient care needs identified from chart review and patient/caregiver interactions.  Must be able to provide clear, concise documentation of care plan to provider through the medical record. This position will aid in creating and completing informative reports, monitoring performance measures and making recommendations as needed. 

Job Requirements

  • Education
    • Required: Successful completion of accredited Practical Nurse program
  • Experience
    • Preferred: 2 years of clinical experience
    • Preferred: Previous experience in one or more of the following-- medical clinic nursing, medical assistant/tech, utilization review, care coordination or discharge planning.
  • Skills
    • Excellent verbal and written communication skills
    • Able to work independently and collaboratively in teams
    • Proficient computer skills
    • Strong problem solving, conflict resolution and negotiating skills
    • Must be able to prioritize, plan and handle multiple tasks simultaneously
    • Proficient computer skills
    • Successful completion of all department specific credentialing within first 90 days of employment
  • Licensure/Certification/Registration
    • Required: MO LPN license
    • Required: BLS must be obtained within 90 days of hire
