Summary

Job Summary
- The Post-Acute Care Services Coordinator will be a part of an interdisciplinary health care team including physicians, advanced practice providers and staff who follow CoxHealth patients in skilled nursing facilities, Cox TCU and REHAB. The coordinator will travel to area Skilled Nursing and Long Term Care Facilities to assist nursing home patients with individual and community needs. The coordinator will define, develop and implement processes to overcome barriers to progression of care for the department. Data analysis, report generation and presentation will be expected. The coordinator will oversee and ensure that triage from the facilities, scheduling for providers and communication is timely and coordinated for patient thru-put. The coordinator will work with the Post-Acute Care Services Director and report to the Clinic Manager of Cox Senior Health. The Post-Acute Care LPN and the Post-Acute Care Coordinator will report directly to the Post-Acute Care Services Coordinator. This position requires daily travel to area SNF/Long Term Care Facilities to coordinate patient care.
