Job Summary

Maintains ongoing contact with health care organizations and medical community to facilitate home care services and streamlines the transition of patient care between facilities and care providers.

RN Job Requirements

Education

Required: Graduate of an approved nursing program

Experience

Required: Minimum one (1) year experience as professional nurse within the last three (3) years.

Preferred: Six (6) months home health experience.

Skills

Knowledge of Medicare and Medicaid regulations desirable.

Licensure/Certification/Registration

Required: MO RN License
