Summary

  • Job Summary
    • Under the supervision of a LCSW the Behavioral Health Consultant (BHC) - MSW provides supportive advocacy to patients and their families as part of an interdisciplinary health care team to overcome barriers to progression-of-care. The BHC works with chronically ill patients enrolled in the Primary Care Health Home program to assist them with management of the behaviors affecting their health. The BHC will work closely with providers, nurse care managers, case management, and other CoxHealth staff, in order to have a patient-centered approach for coordinating the healthcare needs and wellness goals of our patients.
  • Job Requirements
    • Education
      • Required: Master’s Degree in Social Work
    • Experience
      • Required: 2 years of medical or community based social work experience
      • Preferred: Experience providing patient-centered outreach, behavioral health services, needs assessment and support
      • Preferred: Previous experience with electronic health records and prior work in an ambulatory setting
      • Preferred: Experience with Motivational Interviewing, Self-Management and Patient Engagement strategies
    • Skills
      • Excellent telephone and interpersonal communication skills
      • Ability to multi-task and prioritize; strong organizational skills
      • Strong knowledge of medical terminology
      • Computer experience required
      • Demonstrate the ability to work independently, as well as collaborate with interdisciplinary teams and health care providers
      • Comfort with advocating for patients within the medical and community settings
    • Licensure/Certification/Registration
      • Required: LCSW Licensure, LPC, LMFT, or MO State Psychologist License must be obtained within 2 years of start date
