- Job Summary
- The Nurse Care Manager (NCM) position is responsible for facilitating an organized, interdisciplinary team approach to care management to assure a quality, cost-efficient plan for identified patients. Along with other members of the Population Health team, the NCM will conduct comprehensive assessments that include the medical, behavioral, pharmaceutical, and social needs of the patient; identify gaps in care and barriers to attaining improved health. The NCM will assist in developing treatment plans to meet health care goals for individuals, serve as a resource to physicians and staff, and monitor and follow-up on patient’s progress regularly. This position will measure, improve, and maintain quality outcomes, including clinical, financial, and functional, for individual patients and the population served.
- Job Requirements
- Education
- Required: Graduate of an approved nursing program
- Preferred: Bachelor’s Degree in Nursing, Health Sciences or other related field
- Experience
- Preferred: 2-3 years of nursing experience preferably in home health, ambulatory care, community public health, or case management. Knowledge and experience teaching self-management.
- Skills
- Excellent telephone and interpersonal communication skills
- Ability to multi-task and prioritize; strong organizational skills
- Strong knowledge of Medical Terminology
- Computer experience required
- Licensure/Certification/Registration
- Required: MO RN license
- Required: BLS must be obtained within 90 days of hire
