Job Summary
The Care Transitions Navigator maintains ongoing contact with health care organization, Insurance companies, and medical community to facilitate home care services, Specialty Medications, financial assistance programs for medication assistance, and streamlines the transition of patient care between facilities and care providers within CoxHealth 340B clinics. The Navigator will work as a liaison between patients, clinic staff and the Prior Authorization Team. This position will be a great fit for someone who is motivated to be the best for our team and for the patients who need us. it is ideal for someone who wants to be involved in an important system wide initiative to provide quality care in a more cost effective way.
Job Requirements
Education
Required: Graduate of an approved nursing program
Experience
Preferred: Minimum one (1) year experience as professional nurse within the last three (3) years.
Skills
Knowledge of Medicare and Medicaid regulations desirable.
Licensure/Certification/Registration
Required: MO RN License
